Responsibilities: Conduct proactive outbound outreach to encourage patient adherence to Remote Patient Monitoring (RPM) programs and physiological readings. Provide patient education on device usage and collaborate with clinical teams to address barriers to care.

Requirements: Candidates must hold an active Medical Assistant license and possess moderate to advanced proficiency in EHR systems. Strong communication skills and the ability to meet specific home internet and workspace requirements are essential.
