Bilingual/Spanish Speaking Licensed Practical Nurse

Responsibilities: Deliver virtual chronic care support to Medicare patients through proactive outreach, coaching, and goal-setting. Coordinate care to close gaps and document all patient interactions within the Cadence platform.

Requirements: Requires an active compact multi-state LPN/LVN license, fluency in Spanish, and at least 5 years of clinical experience as an LPN. Must be comfortable using AI-assisted clinical tools and working in a remote environment.
