Care Coordinator

Responsibilities: Acts as the primary point of contact for expectant mothers, coordinating care logistics and scheduling visits. Provides non-clinical support by connecting families with community resources and managing member engagement.

Requirements: Requires a high school diploma and 1-2 years of experience as a patient care coordinator or community health worker. Must be bilingual in English and Spanish with at least one year of EHR/EMR experience.
