CareConnect Coordinator

Responsibilities: Acts as the central dispatch hub for urgent care, triaging requests and scheduling in-home visits for clinicians across a multi-state region. Manages real-time logistics and communication between field clinicians, triage providers, and clinical leadership.

Requirements: Requires a high school diploma or GED, with a bachelor's degree and 2+ years of healthcare experience preferred. Must possess strong geographic reasoning skills and proficiency in navigating multiple scheduling and EHR software platforms.
