Licensed Practical Nurse (CA license required) - Hiring in New Mexico

Responsibilities: Deliver virtual chronic care support to Medicare patients through proactive outreach, coaching, and goal-setting. Coordinate care to close gaps, document interactions in the Cadence platform, and collaborate with clinical teams to escalate patient concerns.

Requirements: Requires an active compact multi-state LPN/LVN license in the state of residence and an active CA state LVN license. Must have at least 5 years of clinical experience as an LPN and be equipped for remote work.
