Job Summary

Join the Call4Health care team as a Remote Licensed Practical Nurse (LPN) and make a meaningful impact on patient care from the comfort of your home. In this role, you will support clients by managing Electronic Health Records (EHR) and patient portals, providing patient education, and assisting with Remote Patient Monitoring (RPM) equipment and results. You will also counsel patients with chronic conditions, conduct follow-up outreach between provider visits, and process medication refills in accordance with established protocols.

This position plays a key role in ensuring clear communication between patients and healthcare providers, improving patient outcomes, and helping reduce provider workload through timely, efficient clinical support.
