Medical Assistant

Responsibilities: Deliver virtual chronic care support to Medicare patients through proactive outreach, coaching, and goal-setting. Coordinate care to close gaps and document all patient interactions within the Cadence platform.

Requirements: Requires an active Medical Assistant certification and at least 5 years of experience as a Medical Assistant. Must be remote-ready and proficient in using AI-assisted clinical tools.
