NBHWC Certified Health Coach

Responsibilities: Provide virtual chronic care support and one-on-one coaching to Medicare patients to help them achieve personalized health goals. Coordinate care to close gaps, document interactions in the Cadence platform, and collaborate with clinical teams to escalate patient concerns.

Requirements: Requires an active NBHWC health coaching certification and at least one year of experience as a health coach. Candidates must have experience in chronic condition support and be ready for remote work, with Spanish fluency strongly preferred.
