Nurse Practitioner (Full Time)

Responsibilities: Conduct virtual assessments and medication reviews for patients with suspected dementia to facilitate early diagnosis. Collaborate with care navigators and the Head of Medical to develop streamlined clinical workflows and comprehensive care plans.

Requirements: Requires a Family or Adult-Gerontology Nurse Practitioner license in Arizona, Oregon, or Nevada with at least one year of experience. Must be eligible for licensure in other states and not excluded from federal healthcare programs.
