Nurse Practitioner (Remote, SC License Required)

Responsibilities: Provide comprehensive follow-up care to post-hospital discharge patients via phone and video visits. Coordinate care across hospitals, pharmacies, and community providers while meeting individual and team population health targets.

Requirements: Requires a Certified Nurse Practitioner license in South Carolina with at least 5 years of post-graduate experience in Primary Care or Geriatrics. Must be proficient in digital tools and experienced in managing complex comorbid conditions in Medicare and Medicaid populations.
