Part Time + Weekends Licensed Practical Nurse (CA license required)

Responsibilities: Deliver virtual chronic care support to Medicare patients through proactive outreach, coaching, and goal-setting. Coordinate care to close gaps, document interactions in the Cadence platform, and collaborate with clinical teams to escalate patient concerns.

Requirements: Requires an active CA state LVN license, a compact multi-state LPN/LVN license in the state of residence, and at least 5 years of clinical LPN experience. Must be available to work part-time, including Saturdays, and possess remote work readiness.
