RN Case Manager - Remote

Responsibilities: The Care Manager builds relationships with patients to create personalized care plans addressing medical, behavioral, and social needs. They coordinate care across providers and use data to identify opportunities for improving patient outcomes.

Requirements: Candidates must be a Registered Nurse with at least 5 years of experience, an active Compact RN License, and be local to St. Tamany Parish, LA. Proficiency in care management and comfort with remote technology and multiple EMR systems are required.
