Therapy Continuity Coordinator

Responsibilities: Coordinate the transition of patients from treatment setup to long-term resupply through proactive outreach and retention efforts. Collaborate with clinical and billing teams to track engagement metrics and ensure a seamless resupply experience.

Requirements: Requires 2+ years of experience in healthcare coordination or DME, with a strong preference for PAP/CPAP resupply knowledge. Candidates must be able to work Central Time (CST) business hours and possess strong empathetic communication skills.
