Therapy Continuity Coordinator

Responsibilities: Coordinate the transition of patients from treatment setup to long-term resupply within a digital sleep clinic. Execute proactive outreach and retention workflows to ensure therapy continuity and reduce patient churn.

Requirements: Requires 2+ years of experience in healthcare coordination, DME, or care navigation, with a strong preference for PAP/CPAP resupply knowledge. Must be able to work autonomously in a remote environment during Central Time business hours.
